Lifestyle Medicine Program | NYC Health and Hospitals (H+H)

Health Caregiver

This program gives you access to a team of lifestyle medicine trained experts. Lifestyle Medicine is the use of healthy lifestyle habits to prevent and treat common chronic conditions like type 2 diabetes and high blood pressure.

  • Access a team of lifestyle medicine trained experts including:
    • Doctors and/or nurse practitioners
    • Registered dietitians
    • Health coachs
    • Exercise trainers
    • Psychologists
    • Community health workers
  • The program lasts 6+ months and includes individual appointments and weekly group classes. It teaches you the central tenants of lifestyle medicine which are:
    • nutrition
    • sleep
    • physical activity
    • stress management
    • substance use
    • healthy relationships

Who is eligible

You’re eligible if you’re an NYC H+H patient living with:

  • Type 2 diabetes
  • Prediabetes
  • High blood pressure
  • Heart disease
  • Health concerns related to excess weight
